企画・趣旨のねらい 齊藤利雄（Toshio Saito） 国立病院機構大阪刀根山医療センター 尾方克久（Katsuhisa Ogata） 国立病院機構東埼玉病院 　遺伝性筋疾患は幼小児期に発症する病型が多いが，医療の進歩に伴い寿命が延び，医学的管理を受けながら長く生活するようになった．患者がより良い人生を送るためには，小児期から成人期にかけて適切に医療を移行することが望まれる．患者によっては青年期が，病期による病態の変化で医療の重心が変わる時期にあたり，移行期医療がその絶好の機会となる．幼小児期に発症する神経・筋疾患の診療にあたっては，患者の成長や加齢を念頭に置きながら，疾病の病態特性を踏まえて，小児神経科医と神経内科医の相互理解のもとに連係することが望ましい． 　このシンポジウムでは，遺伝性筋疾患をテーマとして，小児を中心に診る小児神経科医と，成人を中心に診る神経内科医，小児から成人まで診療の対象とする在宅医療とリハビリテーション，そして行政おのおのの立場から，日常診療で望まれる移行期医療について論じる．
