脳卒中・循環器病対策基本法における、循環器疾患回復期医療の位置づけ

心大血管リハビリテーション（心リハ）は循環器各種疾患において予後改善効果が報告されており、日米欧その他すべてのガイドラインで強く推奨されている。急性期病院入院中の心リハ施行率は近年増加傾向にあるものの、その後の回復期における外来心リハ継続がシームレスに実施されていない。一方、2020年に経験した未曾有のコロナ禍では、外来通院自体が困難となり、外来心リハの継続率は著しく低下した。今回のような特殊な事態は別としても、医療機関へのアクセスが困難、もしくは何らかの理由で通院できずに心リハを継続出来ない患者群へ、オンライン心リハは有効な解決策の一つとなることは疑うまでもない。既に海外では、自宅でのオンライン心リハが、医療機関における心リハと同様に安全で効果的であると次々に報告されている。しかし本邦では、2021年に発刊された心リハガイドラインにおいてクラスⅡaに位置しているものの、オンライン心リハは保険適応となっていない。今回のシンポジウムがオンライン心リハ保険収載の実現に向けた足がかりとなることを願い、遠隔医療の技術を生かし必要な人に心リハを提供できる方策を政策的観点も含めて議論したい。
