The relationship between oral function and dental morphology during the mixed dentition period

矯正歯科治療において、側方的問題に苦慮することが多い。通常、良好な顎口腔機能を有する者は、左右対称の比較的幅の広い歯列弓を有していることが知られており、審美的にも優れていると言われている。一方、不正咬合患者には、上顎の狭窄やそれに伴う下顎の偏位や顔面の非対称など側方的問題を有する者が多く含まれている。また、その評価法については、スマイルなどを含む審美的評価と、咀嚼機能等を調べる機能的評価が存在するが、十分なコンセンサスを得られていない評価法も散見される。また、治療法については、狭窄歯列弓に対しては、拡大治療が行われ、顔面非対称症例については、成長期の患者で機能的問題があれば、その改善を行い、成人であれば外科的矯正治療を行うなど、側方的問題に対して様々なアプローチが存在する。そこで、本シンポジウムでは、３名の演者をお招きして、抜歯と側方拡大による非抜歯に関する神話と真実、側方拡大と咀嚼運動との関係、咀嚼トレーニングによる歯列拡大の効果、非対称症例の機能的問題、側方拡大の一つであるMARPEなどについてお話頂き、これまで十分なコンセンサスが得られていなかった側方的問題に対する評価法と治療法などについて少しでも理解が深まれば幸いです。<br />（公益社団法人日本矯正歯科学会　理事、鹿児島大学大学院医歯学総合研究科歯科矯正学分野　教授　宮脇　正一）<br />（公益社団法人日本矯正歯科学会 常務理事、アリビオ矯正歯科クリニック　院長　久保田　雅人）
