低心機能患者の術中術後輸液管理

【座長のことば】心臓血管麻酔では、周術期を通じて心臓外科医、麻酔科医、看護師、臨床工学技士、理学療法士などのチームで質の高い医療を行うことが重要である．しかしながら多くの施設で術後管理は心臓外科医に一任され、診療内容についても施設ごとの＜ローカルルールに沿った医療＞が行われているのが現状である。 術後管理は意識評価、鎮痛鎮静、循環管理、呼吸管理、凝固系評価、電解質補正、血糖コントロール、感染コントロール、早期リハビリテーションなど多岐にわたるが、中心になるのは輸液を含めた循環管理である。 循環管理においても、原疾患、術前の心機能、合併症、術式、麻酔方法、人工心肺、使用薬剤、循環作動薬や輸液に対する反応性、術中経過などを把握した上での一貫した管理が要求される。 本パネルディスカッションでは一般的な周術期管理から一歩踏み入れて、心機能、ERAS、腎機能という３つの視点から周術期輸液管理を再考する．本講演の内容が明日からの臨床のヒントになれば幸いである。
