成人心臓血管手術における自己血輸血の現状：希釈式自己血輸血を中心に

【座長のことば】Patient Blood Management（患者血液管理）とは、“患者の最適な血液管理のために、患者中心のエビデンスをもとに系統立てた医療、そして高い質と効果的な患者ケアを保証するための輸血療法”と定義されている。さて、我々麻酔科医にとって心臓血管外科手術は、手術の性質上『周術期の止血・輸血療法』というキーワードと極めて密接な関係にある。では、“患者中心の適正な輸血療法”を考える時、１）出血を減らすための有効な止血法は？、２）輸血を判断する有益な検査項目は？、３）輸血を開始する時の血液検査のトリガー値は？、４）どの血液製剤の投与を選択するか？、５）どの時点で輸血を終了するか？など多くの疑問点が浮かび上がる。しかし、今後の日本社会と医療の現状とを重ね合わせると、それらの疑問が簡単には解決できないことが想像される。そこで今回のシンポジウムでは、心臓血管外科手術に焦点を当てて、人工心肺における血液希釈、人工心肺中の血液凝固管理、希釈式自己血輸血、本邦における輸血療法の現状、というテーマを麻酔科医、臨床工学技士、輸血部スタッフ間で議論し、心臓外科周術期のPatient Blood Managementについて、未来への解決の道を探っていきたい。
