開心術に伴う心筋傷害の治療標的になりうる分子メカニズム

【座長のことば】心臓手術後の臓器障害は著しく患者のADLを低下させ、生命予後にも大きく影響を及ぼす。合併症がおこれば、医療資源の更なる投入が必要となり、入院期間は長引き、療養型病院への転院を余儀なくされる。このことは本邦で大きな問題となっている医療費の増加にも繋がる。<br />術後の低心拍出症候群にはIABPやPCPSなどの補助が必要となり、ICUでの長期管理が必要となる。低体温循環停止で行われる大血管手術後では肺障害や肺出血のため長期挿管、それに伴う肺炎などが引き起こる。心機能が回復しても中枢神経障害が起これば、著しくADLの低下を招く。<br />1990年代から、虚血再灌流障害やプレコンディショニングの研究が進み、臓器障害のメカニズムがサイトカインや免疫細胞から細胞内シグナル伝達まで基礎研究が進んでいる。<br />そこで今回は心臓、肺、脳障害について３人の演者にご登壇頂き、それぞれの臓器障害のメカニズム、基礎研究から実臨床での臓器保護への橋渡しとなるような研究の紹介から、臨床での取り組みをご紹介いただき、明日からの臨床の参考となるようなシンポジウムとしたい。
