周術期管理に難渋したTGAに対するASO症例

【座長のことば】先天性心疾患の治療方針（生後何日で、どのような手術を行って、どのように周術期管理を行い、二回目の手術はいつ行うか？）は一律ではない。患児の状態によるのはもちろん、小児科・外科・麻酔科・ME・看護師を含むチームの技量・特性・設備の条件も考慮して決める必要がある。小児心臓手術を行っている施設の実態は多様であり、手術件数が多いセンター（high volume center）でのストラテジーをそのまま手術件数が少ない施設（low volume center）に適用できるものではない。 体肺動脈シャントは、術後高肺血流性ショック⇔急性期・慢性期のシャント不全・肺動脈狭窄・低形成の問題を抱えており、施設によってストラテジーが多様である。Jatene手術は、小児心臓手術を始める施設が最初に目標にする新生児一期的根治手術であるが、数々のpitfallがある。 このシンポジウムでは、体肺動脈シャントとJatene手術を題材として、多様な施設構成でのディスカッションを計画した。聴衆の皆様にも積極的に議論に参加いただき、「全ての施設で共通」の教科書的な知識ではなくて、その先の「実際の現場での多様」な部分を感じてもらえば幸いである。いまから小児心臓手術を始めるような施設であっても、何かを持ち帰るものができることを期待している。
