呼吸ケアスタッフが人工呼吸離脱プロトコールに参加するためのゲートウェイを考える

海外では医師以外のスタッフが早期に人工呼吸離脱を進め,患者利益に貢献しているが,わが国では医師以外が離脱を実施するには障壁が多い.一方でわが国には医師が長時間不在になるユニットが多く,そこでは離脱の遅延が治療の妨げになっている.本WGは,離脱を円滑に進めるプロトコール作成を目的に,日本集中治療医学会・日本呼吸療法医学会・日本クリティカルケア看護学会の3団体参加で設立された. 本プロトコールは,あくまでも医師と協調して人工呼吸離脱を進めるが,実践的な部分は現場スタッフがプロトコールを参考に進め,早期離脱を図る.プロトコールは,大多数のユニットで受け入れられる内容を目指し,より安全に実践するための教育面にも配慮した. 本WGは離脱を認定制度化することに反対する.我々は,わが国の現状で患者利益と安全を守るためには,現場のスタッフが協力できる離脱プロトコール作成し教育システムを確立することが,一つの解決策であると信じる.
