Implant treatment in patients who lost teeth due to severe periodontitis

　補綴治療を行う上で全ての基本となるのは，疾病の原因除去と補綴後のリスク回避である．特に欠損補綴治療において，欠損の原因は，歯質ならびに歯周組織の細菌学的な要因と，咬合力や種々のパラファンクションによる力学的な要因が複合していることが多く，これらの原因把握が希薄であれば，長期的に良好な予後は見込めない．しかしながら，インプラントを臨床に積極的に取り入れている歯科医師の多くはインプラント治療自体を主体と考え，本来の治療原則を軽視していることが見受けられる．またインプラント治療後の問題もインプラント中心で考えるゆえ，本質を見誤りかねない．8020運動の推進により，天然歯と共存するインプラント欠損補綴が希求される中，あらためて顎口腔系を包括した一口腔単位の包括的インプラント治療が求められている．そこで本シンポジウムでは，欠損に至った疾病原因を把握するために骨格，歯列の不正，そして，崩壊原因を臨床でどのように診断，評価し，対応をしたら良いかを考察したい．
