知的障害のある患者の意思決定支援

企画趣旨 　病状説明や治療法の選択は患者の意思を尊重し行われる。しかし、知的障害をもつ患者の場合、どこまで病状を理解し判断できるか、患者の意思決定能力を見極め支援することは容易ではない。特に重大な意思決定場面においては、代理意思決定者である家族の迷いや葛藤も多い。本事例は、進行胃がん患者への告知、治療選択をめぐって、患者の家族と関係する医療者が共に悩み考え患者の意思決定を支援しようとした事例である。重度の知的障害者でも、「意思」や「意向」、あるいは「考え」や「気持ち」がある。意思決定ができないと判断する前に、どうすれば意志決定を支援できるかと考えることが「意思決定支援」の大切なポイントであろう。 本事例のディスカッションポイントは、 課題1：知的障害のある患者の意思決定能力をどのように判断したらいいのか 課題2：知的障害者への意思決定支援のあり方について 課題3：代理意思決定者である家族へのケア に焦点をあて、フロアから活発な意見をいただきたい。 川村　三希子 利益相反1 〜 13：該当無し 鈴木　晶子 利益相反1 〜 13：該当無し
