医療機関の機能分化が進むなか、急性期病院と慢性期病院の病病連携では、紹介・転院・フィードバックの場面で、情報伝達だけでなく病状・病いの軌跡・機能障害/活動制限の見立てや、ゴール設定、役割認識などの違いからミスコミュニケーションが生じ、病院間のモヤモヤとして表面化しやすい。本セッションは、JPCA専門医部会若手医師支援部門病院総合医チームとJSHGM若手医師部会のメンバーで、急性期・慢性期それぞれの現場から連携のリアルを共有し、ゲストと共に次世代の病病連携のあり方を考える。退院支援のハウツーではなく、その一歩上流にある臨床思考の断絶や役割認識のズレに焦点を当て、「なぜそう受け取られるのか」「なぜ期待と現実のギャップが生じるのか」を検討する。コメント投稿ツールで現地・オンラインから意見を集め、構造的に生じるすれ違いをno blameで言語化し、明日から実践できる連携のコツを参加者と共創する。
