療養型病院の緩和ケア医からみた緩和ケア相談体制における"壁"

地域包括ケアと緩和ケアの統合委員会

緩和ケアは、これまで主として「がん」を中心に論じられてきましたが、命を脅かす疾患はがんに限られず、また、がん患者であっても終末期まで一貫してがん診療連携拠点病院で療養するとは限りません。こうした現状を踏まえれば、拠点病院等における緩和ケアの充実のみならず、地域全体でこれを支える体制の整備が重要です。そこで今期、時限付き委員会として「地域包括ケアと緩和ケアの統合委員会」が設置され、地域の現場に緩和ケアをいかに浸透させるかについて検討を重ねてきました。その結果、①地域における基本的緩和ケアの充実・普及、②必要時に専門家へ相談できる体制の構築、の 2 点が重要な課題であるとの認識に至りました。本企画では、こうした体制整備を進める上で障壁となる &ldquo; 壁 &rdquo; がどこに存在するのかを多職種の視点から明らかにし、その克服に向けた方策について議論します。
