難治性がん疼痛に対する放射線治療の役割

一般社団法人 日本インターベンショナルラジオロジー学会

転移性骨・軟部腫瘍による疼痛は、がん患者の QOL を大きく損なう難治性症状の一つである。これまで薬物療法および放射線治療が標準的治療として長年行われ、疼痛緩和に重要な役割を果たしてきた。近年、これらに加えて IVRによる低侵襲治療も選択肢として広がっている。動脈塞栓術は以前から保険適用となっており、アブレーション治療も 2021 年にラジオ波焼灼療法（RFA）の適応拡大、2026 年には凍結療法の適応拡大が認められ、比較的早期の疼痛緩和が期待できる治療として注目されている。一方で、これらの治療の適切な位置づけや既存治療との連携は十分に共有されているとは言えない。本セッションでは、日本緩和医療学会と日本 IVR 学会の合同シンポジウムとして、緩和医療、IVR、放射線治療の専門家が集い、各治療の役割と連携のあり方を議論し、患者・医療者双方への認知向上と今後の診療体制の発展について展望する。
