がん終末期の糖尿病マネジメント

一般社団法人日本糖尿病医療学学会

糖尿病医療学は、糖尿病医学を基盤とし、その科学的・技術的発展の利益を糖尿病をもつ人が最大限に享受できるよう支援する学問であり、医療者と患者が協働して寿命と QOL の向上をめざす。近年、糖尿病を持つ人の死因の第 1位は悪性新生物となり、がんとの関係は重要な課題である。<br>一方、がん患者でも糖尿病は主要な併存疾患であり、免疫チェックポイント阻害薬による 1 型糖尿病の発症など、がん治療の経過を通じ血糖管理が必要となる場面は増えている。終末期には多様な病態が重なり、糖尿病治療をどこまで行うかは医学的・倫理的判断を伴う。血糖管理の目標も症状、ADL、予後、患者の価値観や支援体制を踏まえ個別化が求められる。<br>さらに糖尿病医療と緩和ケアの間で血糖管理の考え方に乖離が指摘される一方、持続グルコースモニターなど新技術の活用も期待される。本企画では両領域の視点から、がん患者の血糖管理と終末期の実践について議論する。
