知っておいてほしい神経ブロック ― がん疼痛に対する脊髄鎮痛法の適応と実践

がん治療の進歩や罹病期間の延長に伴い、手術を受けるがん患者や緩和ケア対象患者の周術期管理はいっそう重要になっている。特にオピオイド使用中の患者では、術後の疼痛マネジメントやオピオイドの投与方法に関する判断に高度な専門性が求められることもある。また一方で厚生労働省は、がん疼痛に対する神経ブロックなどの専門的治療の推進を掲げているが、その適応や実践については臨床現場において十分に理解・活用されているとは言い難い。<br>本シンポジウムでは、麻酔科医の立場から、周術期におけるオピオイドの適切な継続・調整・中止に関する考え方と、緩和ケア領域における神経ブロックの有効な活用法について紹介する。患者の生活の質を維持向上させつつ安全な疼痛管理を実現するために、麻酔科医がどのような視点で治療に臨んでいるのかについてお伝えし、今後の緩和ケアスタッフと麻酔科医との間の連携のあり方をともに考える機会としたい。
