心理士のカルテ記載は何を伝えるのか―患者の語りと共有のあいだで―

緩和ケアの現場で、患者・家族からつらい気持ちを聞いたり、精神・心理症状に遭遇することは多い。しかし、その内容や体験をどのようにカルテに記載するかは人それぞれではないだろうか。また、聞いた内容をすべて記載するのか？患者の言葉通りに記載するのか、それとも一般化して記載をするのか？患者のプライマリースタッフへの不満についても記載するのか？カルテを読んだ医療者の行動変容を促す意図をもって記載するのか？患者には開示しない前提のアセスメントを記載するのか？カルテ開示に耐えうる内容に限定して記載するのか？などなど、実臨床で悩むことも多い。本シンポジウムでは、精神腫瘍医、心理職、看護師それぞれの立場でエキスパートの立場から臨床実践を紹介してもらい、参加者の日常臨床に役立ててもらうことを目的とする。
