在宅緩和ケアにおける持続皮下投与の安定継続に向けた薬局薬剤師の役割と多職種連携

在宅医療における注射薬療法は、携帯型ポンプの普及とエビデンスの蓄積により急速に拡大し、がんに限らず生命を脅かす疾患に対する重要な治療・緩和手段となっている。しかしその一方で、デバイス選択やコスト、供給体制、さらには倫理的意思決定に至るまで多層的な課題が顕在化している。がん疼痛管理においては PCA ポンプの有用性が示されているものの、医療者の習熟度や教育体制の不足、コスト面の課題から十分に普及していない。さらに、心不全に対する持続静注療法では高精度デバイスを用いた厳密な管理に加え、中断を含めた倫理的な意思決定の困難さが臨床現場の大きな負担となっている。加えて、医療用麻薬の安定供給や在庫管理、患者の生活に即したデバイス運用には多職種連携が不可欠である。本企画では多職種が各領域での実践を横断的に提示し、エビデンスと経験知を統合することで、在宅における安全で持続可能な注射療法体制を構築していけるような議論としたい
