希望に応える医療からの離脱 ― 緩和ケアにおける役割の再定義

がん医療の経過の中で、患者さんが医療者から見ると実現が難しいと思われる希望を持ち続ける場面はしばしばあります。たとえば、進行がんで根治が難しい状況でも「治るはず」と願う、衰弱が進む中でも「食べたい」「歩きたい」「故郷に戻りたい」と願うといった場面です。その希望は、治療や療養の選択、緩和ケアへの移行に影響を及ぼし、ときに医療者を悩ませます。なぜこのような「希望」が生じるのでしょうか。それは誰にとって、どのような問題なのでしょうか。困っているのは患者さん自身というより、対応や説明に悩む私たち医療者の側ではないでしょうか。本セッションでは、「現状と乖離した希望」をめぐる問題を丁寧に捉え直し、患者さんの尊厳を尊重しながら、医療者が何を支え、どのように関わるべきかについて多角的に議論したいと思います。
