ICIによる免疫関連有害事象が示す緩和ケアへの新たな臨床的視点

公益社団法人日本臨床腫瘍学会

「がん治療と緩和ケアの統合」が世界標準となっている昨今、がん治療医と緩和ケア医はそれぞれの専門性を活かした医療を患者に提供しているが、緩和ケア医であってもがん治療について一定レベルの知識が必要である。特に近年のがん治療に伴う副作用の中には、古典的な嘔気・嘔吐、骨髄抑制などに限らず多様な症状が含まれる。免疫チェックポイント阻害剤に関連した有害事象（irAE）や抗体製剤によって生じるサイトカイン放出症候群（CRS）、ICANSと呼ばれる神経障害などが代表的だが、発熱性好中球減少で用いるセフェピムで脳症をきたすことを知らなければ、単に感染症によるせん妄として対処してしまうかもしれない。本シンポジウムでは、がん治療の第一線で活躍する演者から、多くの学びを得ていただけると幸いである。
