急性期医療における心不全緩和ケアの実際と葛藤 ― 看護師の視点から ―

日本心不全学会が提供する HEPT（心不全緩和ケアトレーニングコース）は、今や心不全診療に不可欠な共通言語となっている。本シンポジウムは、日本緩和医療学会との合同セッションとして、この HEPT の知見を集中治療領域（ICU/CCU）という最も過酷な臨床現場に適用するためのチャレンジである。 救命が優先される環境下で、緩和ケアチームはいかに介入し、多職種はどう連携すべきか。循環器医、緩和ケア医、看護師の三者が、現場の困難さと具体的打開策を明らかにする。ICU での意思決定支援から、退院後を見据えた移行期ケアまで、多職種が明日から実践できる「心不全緩和ケアの本質」を議論し尽くしたい。
