難治性疼痛の患者の自宅療養を支えるための多職種アプローチ

在宅療養中のがん患者における難治性疼痛は、単なる薬剤調整の難しさにとどまらず、患者と家族の生活そのものを揺るがす問題である。限られたリソースと環境の中で、私たちはどこまでできるのか。<br>本セッションでは、在宅医療の最前線で難治性疼痛に向き合う医師・看護師・薬剤師が、複雑な要因が絡み合い、オピオイドや鎮痛補助薬の調整のみでは十分な鎮痛が得られない疼痛に対し、どのように向き合い、葛藤し、試行錯誤しているのかを共有する。アセスメント、薬物療法、非薬物的介入、ケアの工夫に加え、患者・家族の揺れ動く思いや生活背景に寄り添った関わり、意思決定支援の実際に迫る。<br>さらに、地域や病院との連携における課題と解決策を議論し、「在宅でもここまでできる」という実践知とともに、「生活の中で満足できる鎮痛」を実現するための視点を持ち帰る機会としたい。
