死亡直前の薬物治療の考え方

死亡直前期には、疼痛、呼吸困難、せん妄など複数の症状が同時に出現し、治療やケアに難渋する場面が少なくない。近年、国内の緩和ケア病棟を対象とした多施設共同研究から、これら混合症状の実態と臨床課題を示す報告が相次いでいる。<br>本パネルディスカッションでは、まず医師の立場から最新の研究知見を共有し、苦痛と意識のバランスを評価しながら、オピオイド、抗精神病薬、鎮静を含めた治療戦略をどのように組み立てるかを具体的に提示する。次に、薬剤師の立場から、臓器機能が低下する死亡直前期における主要薬剤の体内動態の変化と、安全かつ適切な薬物調整の要点を整理する。さらに看護師の視点より、オピオイドのタイトレーションを現場にどう伝えるか、変動する意識レベルの中で患者・家族の対話をどう支えるか、鎮静についていかに説明し共有するかを紹介する。<br>本セッションを通じて、日常診療に直結する実践的示唆を持ち帰っていただきたい。
