抗精神病薬2剤併用のその先は？

統合失調症の薬物療法において抗精神病薬の2剤併用は、真のアウトカムである治療失敗のリスク低減を指標としたスウェーデンの国家規模のリアルワールドデータ解析でクロザピンに次ぎ第2位、持効性抗精神病薬注射より上位に位置していた（Tiihonen J et al. JAMA Psychiatry 2017;74:686）。日本精神科救急学会のJAST study groupによるコホート研究でも、治療失敗を指標に退院後1年間追跡した結果、2剤併用は有意にそのリスクを減じていた（Hatta et al. Asian J Psychiatry, 2022;67:102917）。2剤併用を危険視する理由の副作用面でも、フィンランドの国家規模データベースにおいて、2剤併用は単剤より用量によっては身体疾患での入院や心血管疾患での入院リスクが低いことが明らかにされている（Taipale et al. Am J Psychiatry 2023;180:377）。単剤原則は揺るぎないが、効果が得られない場合の2剤併用は、今や根拠のある臨床現場の技法と言えよう。しかし、強いドーパミン遮断が長期間に及べば、ドーパミン過感受性精神病機序の進展が懸念される。本シンポジウムでは、やむを得ず始めた2剤併用を寛解後に単剤化できるのか？できるとすればいつ、どのようにするのか？あるいは単剤化する必要はないのか？といった現場の切実な疑問を議論したい。本シンポジウムは、日本精神科救急学会の推薦を受けている。
