災害時における子どものリエゾン診療の実際

児童青年期の精神科診療を必要とする子どもは、心身に様々な併存障害をもつケースが多い。また、身体疾患をもつ子どもは、こころのケアも必要となる場合も多い。<br>児童精神科医が勤務する医療施設は、自治体病院や大学病院など地域における基幹病院的な役割を担うことが多く、児童青年期の精神疾患と身体疾患・身体症状を併存する病態は多岐にわたる。摂食障害や身体化障害、てんかん、せん妄、先天性の消化器や循環器系、神経内分泌系疾患、小児腫瘍といった身体疾患に伴う精神症状など、子ども本人がもつケースもあれば、家族に重篤な身体疾患をもつ場合もあり、複雑な家庭内力動のもと、コンサルテーション・リエゾンも小児科、小児外科、産婦人科、救急科など院内外の様々な科にまたがり、訪問看護、特別支援教育、福祉、保健など、様々な地域連携の在り方が求められ、これらの連携はライフステージによっても異なる。<br> コンサルテーション・リエゾンを円滑に進められれば、児童精神科の有用性、存在意義を施設内外の関係者が認識することにもつながる。本シンポジウムでは、多様な医療機関に勤務するシンポジストが、身体の臓器・疾患、シチュエーション別に児童青年期のコンサルテーション・リエゾンの取組について発表し、その課題や今後の在り方について討議する。コンサルテーション・リエゾン精神医学は古くからあるテーマで、医学の進歩に伴い変化していき、子どもを取り巻く状況の変化に応じて柔軟に対応していくことが望まれる。
