Treatment strategies for NMOSD based on  the neuropathological findings  

視神経脊髄炎スペクトラム障害 (Neuromyelitis optica spectrum disorder: NMOSD)は、疾患特異的自己抗体であるアクアポリン４（AQP4）抗体の発見により、病態理解が加速度的に進んだ疾患である。AQP4は中枢神経内において、主にアストロサイトの足突起に発現しており、一次的な攻撃対象と考えらるが、NMOSDの急性期における神経病理学的所見は、アストロサイトの広範な消失と相対的な髄鞘保存により特徴づけられる。また、アストロサイト障害病変内の血管周囲に活性補体の沈着が、グリア限界膜あるいはアストロサイトの足突起に沿って認められることから、補体介在性のアストロサイト破壊性障害が、NMOSDの本質的な病態であると理解される。そのため、補体活性の制御は、NMOSDの治療において非常に重要な役割を持つ。本講演では、主に神経病理学的所見に基づいたNMOSDの治療戦略について概説する。
