Italian Law 219/2017 emphasizes the importance of communication between doctor and patient in the treatment process and promotes the involvement of family members in therapeutic decisions too. Moreover, the loss of freedom of choice of the modern and postmodern person due to the pressures of the economic system is discussed. The need to value collective capital and to consider the historical and social context in which we live in order to understand current human conditions is highlighted. In addition, the importance of a complex and integrated vision in the field of social psychotherapy is noted in order to address human suffering more effectively. Giving people information about possible treatments increases their possibilities of choice and participation in treatment pathways, which in a dialogical approach can include different methods from recovery to care budget. In this way, it is possible to deal with difficult sufferingand existential situations, such as those faced by people diagnosed with borderline personality disorder. The tools used by our team in the Metropolitan area of Milan – with almost 5 millions inhabitant concentrated in a limited space - are reported, which include integrated pathways starting from crisis situations to hospitality in therapeutic communities (with different levels of protection and rehabilitation intensity), home care, psychotherapeutic treatment and territorial rehabilitation. Short interviews of interested people, their family members and their interventions during the stakeholder days are made available; clinical cases are examined. A dream analysis of an young immigrant woman from East-Europe will be also showed and analyzed as a way of systemic therapeutic treatment
