Assoc Prof Kheng Hock Lee is a family physician and a pioneer of integrated care in Singapore. With a clinical foundation in family medicine, he led the development of Singapore General Hospital’s transitional home care and introduced social prescribing in 2019. Currently serving as Deputy CEO (Academic Medicine and Partnerships) at SingHealth Community Hospitals, he also directs the world's first WHO Collaborating Centre for Social Prescribing. A Past President of the College of Family Physicians Singapore, he remains dedicated to bridging the gap between clinical practice and community-based social health to achieve holistic population wellness.
