【概要】Plasma p-tau217 and the p-tau217/Aβ42 ratio detect Alzheimer’s pathology with accuracy approaching CSF, but the interpretation differs compared with CSF and PET.　This talk covers these blood biomarkers for Alzheimer’s disease, the rationale for a two-cutoff approach that could limit confirmatory testing to the intermediate zone, and how to handle intermediate results. It also presents the updated Alzheimer’s Association clinical practice guideline and the gap in diagnostic accuracy between memory clinics and primary care, where most patients are first assessed.
