In line with the theme of the 35th Annual Meeting of the Japanese Society of Pharmaceutical Health Care and Sciences, this international session will explore how clinical pharmacy is being deepened and expanded across global health care systems to improve patient outcomes. Experts from various countries will share their latest efforts in advancing pharmaceutical care through innovations such as personalized medicine, including therapeutic drug monitoring (TDM), the integration of cutting-edge technologies and digital transformation, interdisciplinary collaboration, and pharmacist-led interventions. The session will provide insights into how these strategies ultimately enhance the safety and effectiveness of pharmacotherapy. Through international exchange, we aim to foster new directions in pharmaceutical health care that address both local and global challenges.
