[SY-92-02]Implementing social prescribing in the nationwide dementia care and prevention initiative in China

The growing population in old age has put great challenges in mental health. However, the response to mental healthcare needs is often fragmented and poorly coordinated. From the view of a complete state model of health, providing continuous holistic care for older adults is necessary to address mental health challenges. Social prescribing has emerged as an approach that enables the link between medical and social care. The feasibility to implement social prescribing for promoting dementia care and prevention will be presented. Also, the preliminary findings of social prescribing in improving older adults’ cognitive performance will be discussed.
