Background Until now, NIPT has only been available for high risk pregnant women (e.g., aged 35 years or older) in Japan. In 2022, the Japanese Medical Association issued a guideline stating that for pregnant women whose concerns are not resolved after genetic counseling (GC), their decision to undergo NIPT should be respected regardless of their age. Kansai Medical University Hospital (KMUH) plays a central role in perinatal care in the Kitakawachi area of Osaka and provides both prenatal screening and diagnostic tests. To help establish a better GC system, we conducted a retrospective survey of GC visits among low to middle risk pregnant women. Method Pregnant women aged 34 years and younger who visited KMUH for the first time between May 2016 and December 2021 at a gestational age of fewer than 18 weeks were included in the study. Gestational age, GC visit, the reason for the visit, and choice of the test after GC was examined using medical records. Results Of 1,669 who met the criteria, 360 underwent GC. 20% underwent GC for reasons other than the previous detection of fetal or chromosomal abnormality. After GC, 83% requested a test. Their choice of the test included first-trimester combined test (73%), first-trimester ultrasound screen (17%), and NIPT (9%). Discussion Twenty percent of low to middle risk pregnant women underwent GC, and 83% of them requested prenatal tests. It is necessary to establish a system that meets the needs of pregnant women of all age groups as information on prenatal testing begins to be provided to all pregnant women.
