[P16-8]当院における羊水検査の現況と課題

With the revision of the policy of Japanese Ministry of Health, moving to expand accreditation of non-invasive prenatal testing (NIPT) facilities, the need for prenatal diagnosis is increasing. Future introduction of NIPT is discussed at our hospital, it is necessary to present the option of amniotic fluid testing, and understand the pregnancy outcome in positive test cases. We retrospectively extracted 102 cases in which amniotic fluid testing was performed at our hospital using electric chart records, and distribution of maternal age, results of chromosome analysis and pregnancy outcome were examined. The indications for amniotic fluid testing included 21 elderly pregnancies, 63 with noted fetal ultrasound abnormalities, 9 with a history of chromosomal abnormal pregnancy, and 9 other cases.Amniocentesis was successfully achieved except for one case. After amniocentesis, 10 patients (9.9%) required extended hospitalization due to increased uterine contraction, but no miscarriages were experienced. Eighteen women had positive chromosome tests. There were no positive cases for elderly pregnancies with no apparent abnormalities on ultrasound. In terms of pregnancy outcome, four cases of the positive test continued pregnancy followed by uneventful delivery. Eight cases opted to have elective abortion, and 6 cases with 18 trisomy resulted in intrauterine fetal death or neonatal death.The fact that no positive amniotic fluid test cases were found for elderly pregnancies without ultrasound abnormalities may help to present the option of NIPT for such cases.
