Many AI deployments stall after pilot phases because a formal continuation review is not initiated. Using the adoption of communication and healing devices in care facilities as a motivating context within the care sector, this paper shifts the focus of “continuation” from contract signing to an earlier point at which an issue is placed on management’s agenda, a decision owner is assigned, a deadline is set, and reporting requirements are defined. We introduce a two-lane model in which alignment between frontline effectiveness (“it works in practice”) and managerial justification (“it can be explained and governed”) is necessary; misalignment can produce a continuation gap. Based on patterns observed in publicly available cases, agenda-setting during trials often becomes observable before formal pricing discussions, suggesting a more proximal indicator of continuation readiness. We define strategic co-creation as the collaborative design of “decisionability”—operability, evaluability, and accountability—during the trial phase. This approach clarifies how pilot trials can be connected to formal reviews and ongoing use, aligning with Creating Shared Value (CSV). Future studies will test the proposed mechanism using operational logs and interviews.
