A formula is made the medicine optimal for each patient in proper quantities to use the medical supplies safely, and a patient takes it right and moreover a patient/family/medical person and others need to monitor an adverse event in relation to dosing without a break. The separation of pharmacy and clinic by the publication of the prescription of the outside of hospital develops in recent years, and a system is built that a hospital and community pharmacists takes local resident's drug treatment, cooperating, like the main constituent. A social aging moves ahead, and the area cooperation is regarded as being more important increasingly to solve an accumulated problem such as polypharmacy to multi-comorbidity of patient. In this symposium, we introduce the effort that improves the safety of the medication from the view point of hospital/community pharmacy each by the information cooperation and want to try of that problem to think of, sharing the life environment of a patient and family and thought about a drug-treatment how, it is better to build the cooperation between a patient/medical person or between the numerous types of jobs of a doctor/nurse/pharmacist and others about something. It introduces the effort of area FORMULARY lastly.
