PCIにおける造影剤量低減の現実と挑戦

心血管疾患 (CVD) を持つ患者は慢性腎臓病 (CKD) を併存していることも多く、臨床において腎機能低下症例を経験します。造影剤腎症は、造影剤の投与後に発症する急性腎障害で、腎臓の機能が一時的に悪化する状態です。造影剤腎症は一過性であり、機能は回復することが多いとされていますが、造影剤腎症を1度発症すると予後が悪くなります。造影剤は検査や治療に必要なツールの1つですが、腎機能低下などのリスク因子を有する患者に造影剤の使用を避けられない状況があリます。今回、PCI、EVT、Structure(大血管)を行う上でどのように造影剤を減量して手技を行うことができるのか、施設での取り組みや方法などを共有できればと思います。
