[FPoIQwB5-02]Prevalence and contributing factors of Postnatal depressive symptom in rural Tanzania: Evidence from health-center

Background: Postpartum depression (PPD) is a global maternal health problem affecting about 1 in 5 mothers after delivery and can adversely impact both mother and newborn if not properly addressed. The global prevalence of PPD is 19.18% but higher rates in developing countries. There is still lack of epidemiological evidence regarding PPD in Tanzania. Therefore, this study aimed to assess the prevalence and associated factors of PPD among mothers in rural Tanzania.
Method: This study is a health-center based cross sectional analysis including 763 postpartum mothers from the Pwani region of Tanzania. This study was conducted between December 2023 to September 2024. Four health centers from Kisarawe and Bagamoyo districts were selected randomly. Data was collected by structured questionnaire. Postpartum depressive symptoms were measured by administering 10-item Edinburgh Postnatal Depression Scale (EPDS). A cut of value >=10 was used to indicate the presence of postpartum depressive symptoms. Data analysis was conducted in STATA 18.5 software. Descriptive data were analyzed using frequencies and percentages. Univariate regression analysis was performed to identify the associations between dependent variable and independent variables, with a p value <0.05 was considered as a statistically significant.
Results: Among 763 participants the overall prevalence of postpartum depressive symptoms was 19.79%. The mean age of the mothers was 27.55 years; most were married (71%) and literate (93%). Over half were employed (60%), two-thirds owned family assets (68%), and 75% reported a monthly household income above 5,000 Tanzanian shillings (>2 USD). In the univariate analysis, mothers with >= 35 years of age (OR:1.67, 95% Cl:1.04-2.68; P=0.035), and unmarried mothers (OR:1.75; 95% Cl:1.06-2.90; P=0.029) were associated with PPD.
Conclusion: The prevalence of postpartum depressive symptoms among mother in the Pwani region of Tanzania is similar with the global prevalence. Unmarried and older mothers were associated with developing PPD. Implementation of routine mental health screening during postnatal checkups might help in reducing postpartum depressive symptoms.
