[ctkh3Px5-03]Gender difference in Greenness effect on preventable mortality among middle-aged people: evidence from 47 cities in Japan, 2001-2018

[Purpose]The protective effect of greenness on reducing human mortality is well recognized. However, the gender difference in greenness on preventable mortality among middle-aged adults is still unclear.
[Method]To address this deficit, we conducted a study on middle-aged adults (40-64 years) across 47 cities in Japan. We measured the normalized difference vegetation index (NDVI) to reflect the greenness level from 2001 to 2018. We calculated the greenness and all-cause and cause-specific mortality associations, stratified by gender levels. Moreover, we computed the gender difference of preventable deaths attributed to greenness changes and differences in 2001-2010 and 2011-2018.
[Results]The result shows that a 0.1 increase in NDVI associated with an all-cause mortality rate ratio (MRR) of 0.96 (95% CI: 0.93-0.98) in males, compared with 0.97 (95% CI: 0.95-0.99) in females. A 0.1 increase in NDVI corresponded to a 7% decrease in respiratory mortality in males (MRR= 0.93, 95% CI: 0.89-0.97), compared to a 6% decrease in females (MRR= 0.97, 95% CI: 0.90-0.98). The changes of NDVI from 2001 to 2010 and from 2011 to 2018 could avert 79 and 7 all-cause deaths among females, respectively, compared to 174 and 13 deaths among males. If the NDVI reached the counterfactual level in 2001 and 2010, it could reduce all-cause mortality by 985 and 828 deaths among females, respectively, compared to 2,251 and 1,935 deaths among males.
[Consideration] Although men have slightly higher preventable effects compared with women, we observed evidence that increased greenness reduces the mortality both genders.
[Conclusion]These findings indicate the potential of a population-level intervention for reducing mortality in Japan, which has important public health and social equality at the city level to achieve SDG5.
