Application of NAMs to Drug Safety Assessment and Perspectives on ICH S7A Guideline Revision

Advances in cancer therapy have improved long-term survival; however, the increasing incidence of cancer therapy–related cardiovascular toxicity has become a major concern. Diverse cardiotoxicities associated with anthracyclines, molecularly targeted agents, and immune checkpoint inhibitors have been reported, yet the systematic collection of cardiovascular events in oncology trials remains insufficient. Treatment modifications and the presence of late-onset cardiotoxicity further complicate clinical assessment. Evidence regarding the efficacy and safety of cardioprotective agents, as well as their interactions with anticancer drugs, also remains limited. These clinical challenges highlight the need touse New Approach Methodologies in non-clinical testing.<br />   This symposium will discuss how clinical needs can be effectively integrated into preclinical evaluations. Key topics include mitochondrial and structural toxicity assessment using human iPSC-derived cardiomyocytes, characteristic toxicity profiles of anticancer drug classes, and the role of intracellular quality control mechanisms. We will also address data-driven approaches that combine real-world data, pharmacokinetic and genomic information, and animal studies, applying machine learning to identify cardiotoxicity risk factors.<br />   Thus, by bridging basic science, clinical practice, and regulatory science, this symposium aims to support safer cancer therapy through improved prediction and prevention of anticancer drug-induced cardiotoxicity.
